Description:

Provides wraparound services to high-risk youth and their families by supporting young adults who are at risk of being placed out of the home due to the severity of mental health behaviors and reoccurring substance abuse disorder. The goal of the program is to prevent clients with existing family support system from entering a juvenile detention facility.

Hours:

Monday-Thursday: 07:30 am-07:00 pm-Friday: 07:30 am-05:00 pm.

Accepts most major forms of insurance. An income based sliding scale is available.

Eligibility:

Serves Duval County youth ages four to 21. Client are generally at risk of out-of-home placement, involved with the Juvenile Justice System, have had multiple episodes involving law enforcement, show poor academic performance and/suspensions, and/or had two or more psychiatric hospitalizations in the last 12 months.
