Provides a comprehensive case management program aimed at assisting children and adolescents with serious emotional disturbances. This program includes individualized support to help clients and their families access necessary medical, social, educational, and other essential services with the goal of avoiding out-of-home placements, such as juvenile detention or residential treatment.

Hours:

Monday-Thursday: 07:30 am-07:00 pm, Friday: 07:30 am-05:00 pm

Intake Process:

Scheduled by the case manager and parent or guardian.

Program Fees:

Call for information.

Eligibility:

Serves residents of Duval County age four through high school graduation.
