Eligibility:

Serves Duval County residents. Pregnant or breastfeeding women, and/or with infants and children (infant to five years of age) currently receiving Medicaid, SNAP, or TANF
First-time applicants must provide photo ID, baby's crib card, shot record, SS for all household members, proof of income, if not receiving Medicaid, TANF, SNAP, pay stubs and proof of address (utility bill or lease), proof of income (if not on Medicaid, TANF, and/or SNAP), proof of current address (driver's license, utility bill).
