Description:

Program is designed to assist seniors living independently, with or without caregiver, to navigate the spectrum of local older adult services. Staff work directly with the senior to develop a care plan that meets the client specific needs, and is designed to help the client live independently. Program also offers families and caregivers support, education, and referrals to external resources related to the senior's continued physical, emotional, mental, and financial health.

Hours:

Monday-Friday: 09:00am-05:00pm.

Serves older adults age 60 or older living in Baker, Clay, Duval, Nassau, Putnam, Saint Johns, as well as their caretakers.
