Description:

Provides nutrition education and food vouchers for pregnant women, new mothers, infants and children younger than age five who cannot afford an adequate diet and have a condition which shows a need for better nutrition.

Hours:

Monday-Friday: 07:00 am-05:00 pm.

Intake Process:

Call for an appointment or before walking in (walk-in times vary and are subject to change).

None.

English and other languages.

Eligibility:

Pregnant or breastfeeding women, and/or with infants and children (infant to five years of age) currently receiving Medicaid, SNAP, or TANF
First-time applicants must provide photo ID, baby's crib card, shot record, SS for all household members, proof of income, if not receiving Medicaid, TANF, SNAP, pay stubs and proof of address (utility bill or lease), proof of income (if not on Medicaid, TANF, and/or SNAP), proof of current address (driver's license, utility bill).
