Provides a variety of psychiatric outpatient services for adults, including standard and intensive case management, as well as medication management.

Hours:

Monday-Tuesday: 08:00 am-04:30 pm, Wednesday: 08:00 am-06:00 pm, and Thursday-Friday: 08:00 am-04:30 pm.

Intake Process:

Call to complete an initial intake screening.

Program Fees:

Most insurances are accepted for psychiatric services and medication management, including Medicaid and Medicare. Case Management services are covered by Medicaid.
An income-based sliding scale is available.

Languages:

English; other language interpreter service available upon request.

Eligibility:

Adults age 18 or older with chronic mental illness residing in Baker, Clay, Duval, Nassau or Saint James County.
