Hours:

Monday-Friday: 08:00am-04:30pm.

Intake Process:

Call for an appointment.
A list of required documents will be provided during initial phone intake.

None.

Languages:

English.

Eligibility:

Must be Ryan White eligible, including diagnosis with HIV/AIDS with limited income. Must reside in Duval County.
