Hours:
Monday-Friday: 07:00 am to 04:00 pm.
Intake Process:
Call for initial intake screening.
Program Fees:
Accepts most insurance, including Medicaid and Medicare. An income-based sliding scale is available.
Languages:
English; other language interpreter service and ASL (for hearing impaired).
Eligibility:
Individuals 18 years of age or older with chronic mental illness residing in Baker, Clay, Duval, Nassau, or Saint Johns county.
