Description:

This is an outpatient facility that provide basic health care including physical examinations, immunizations, family planning, nutrition assistance and diagnosis and treatment of common ailments for low income people. They provide no cost medication, medical case management, health education classes, vision screenings, low cost eye glasses and wheelchairs. Appointments are necessary.

Hours:

Mon-Tue - 8:00am to 4:30pm
Wed - 8:00am to 7:00pm
Thu-Fri - 8:00am to 4:30pm

Sliding scale
Valid government-issued ID
Copy of pay-stubs / four weeks, if applicable
Letter of support from a shelter, if unemployed

None, if qualified (under-insured or no insurance)

Eligibility:

Uninsured homeless or at risk of being homeless
Income at or below 200 percent of Federal Poverty Guideline for the current year
Duval County resident
18 years of age and above
Beaches clinic serves 32233, 32250, 32224, 32266, 32082 only
