Provides variety of outpatient psychiatric services for adults, including mental health screening, assessment, and therapy; substance abuse screening, assessment, and therapy; psychiatric evaluations; medication management; and care coordination.

Hours:

Monday-Friday: 08:00 am-05:00 pm

Intake Process:

Call for initial intake.

Program Fees:

Most insurance accepted, including Medicaid and Medicare. An income-based sliding scale is available.

Languages:

English; other language interpreter service available as needed.

Eligibility:

Serves individuals age 18 or older who reside in Clay, Baker, Duval, Nassau, or Saint Johns counties.
