Provides basic outpatient health care including physical examinations, immunizations, family planning, nutrition assistance and diagnosis and treatment of common ailments. Also offers no-cost medication, medical case management, health education classes, vision screenings, and low-cost eye glasses and wheelchairs.
Hours:
Monday-Tuesday: 08:00am-04:30pm , Wednesday: 08:00am-07:00pm, and Thursday-Friday: 08:00am-04:30pm

Sliding scale based on income and insurance.

Eligibility:
Serves residents of Duval County. Proof of residency required.
Must be 18 or older with income at or below 200 percent of Federal Poverty Guideline (FPL) for the current year, based on copies of pay-stubs for the prior four weeks. If unemployed, requires a letter of support from a homeless shelter.
Targets uninsured homeless or those at risk of becoming homeless.
