Description:

Provides clinical services to help individuals living independently in the community perform activities of daily living. Services include an individualized assessment, service planning, linking and referring to external supports, monitoring, advocating, coordinating services, 24-hour on-call coverage, crisis support, and skills training.

Hours:

Monday-Friday: 08:00 am-05:00 pm

Intake Process:

Call for initial intake. Once, enrolled, most services are conducted in the client's home.

Program Fees:

Accepts most insurance, including Medicaid and Medicare. An income-based sliding scale is available.

Languages:

English; other language interpreters available as needed.

Eligibility:

Individuals 18 or older with a diagnosis of severe and persistent mental illness who require ongoing clinical support to remain living independently. Serves residents of Baker, Clay, Duvall, Nassau, and Saint Johns counties.
