Description:

Provides a transition program for adults who have not previously been able to live independently in the community. All persons enrolled in the program reside with an adult foster family and receive intensive targeted case management services.

Hours:

Monday-Friday: 08:00 am to 05:00 pm.

Intake Process:

Call for initial intake.

Program Fees:

Case management services covered by Medicaid; call for additional information.

Languages:

English; other language interpreter service available as needed.

Eligibility:

The program serves adults age 18 or older with severe and persistent mental illness transitioning our of a clinical inpatient environment. Must reside in Baker, Clay, Duval, Nassau, or Saint James county.
