Provides nutrition education and food vouchers for pregnant women, new mothers, infants and children younger than age five who cannot afford an adequate diet. This program specialize in the care of the teeth which includes the prevention, diagnosis, and treatment of diseases of the teeth and gums.

Hours:

Monday: 08:00 am-05:00 pm, Tuesday: 08:00am-07:30pm, Wednesday-Friday: 08:00 am to 05:00 pm.

Intake Process:

Call for an appointment.

Program Fees:

Accepts most insurance, including Medicaid and Medicare. An income-based sliding scale is available.

Languages:

English and Spanish.

Serves Duval County residents.
