Provides immunizations for a variety of childhood diseases such as tetanus (lockjaw), diphtheria, measles, mumps, chicken pox (varicella), rubella (German measles), polio and hepatitis A and B, some of which are typically mild in children but generally more serious when contracted by adults.

Hours:

Monday-Friday: 08:00 am-12:00 Noon and 01:00 pm-05:00 pm.

Call for an appointment.

Program Fees:

Accepts most insurance, including Medicaid and Medicare. An income-based sliding scale is available.

Languages:

English and other languages.

Eligibility:

Child under age 19 and resident of Florida. Must provide current vaccination card.
