Description:

Maintains access to birth certification records for individuals born in Florida and death certification records that occurred in the State of Florida. Provides statistics which relate to service utilization patterns within the community and provide data regarding duplication of service and service gaps for use by public and private planning bodies.

Eligibility:

For a birth certificate:
Individuals, parents or guardians who present court documents showing legal custody. Attorneys representing children and adolescents under 18; documentation is required.
To obtain a death certificate:
No restrictions to obtain a copy of a death certificate record without the cause of death showing on the certificate. To obtain a death certificate showing cause of death only a family member or a beneficiary of a deceased person's life insurance policy or an estate administrator; documentation is required.
