Florida's First Coast YMCA Diabetes Prevention Program

Description:

Provides education and training in self-management and lifestyle techniques to individuals who have been diagnosed with prediabetes, or believe they may be at high risk of developing type 2 diabetes. Services include learning about healthier eating, physical activity, and other behavior changes. The program is generally one-year long and is broken down into 16 one-hour weekly sessions followed by bi-weekly and monthly sessions for added support during the year.

Monday-Friday: 08:00 am-05:00 pm.

Program Fees:

Fixed fee. Program is covered by most insurance plans, and financial assistance may be available for the un- or underinsured. YMCA membership is not required.

Eligibility:

Participants must be 18 years of age or older, overweight/obese (having Body Mass Index [BMI] greater than 25) and be at high risk for developing type 2 diabetes, including those who have already been diagnosed with prediabetes.
