Description:
Provides comprehensive medical services to uninsured patients. These services include orthopedics, oncology, dermatology, cardiology, urology, gastroenterology and others. Additional services include chronic disease management, vision screening, hearing screening, and Gerri!s Corner Health
Hours:
Monday, Tuesday, and Thursday: 08:00 am-05:00 pm, Wednesday: 08:00 am-12:00 Noon, Friday: 08:00am to 02:00pm.
Intake Process:
Must complete application to receive ongoing service. Call for more information or walk in for urgent care services.
Program Fees:
Some services are Medicaid-eligible. Sliding scale available. Medicare and private insurance not accepted.
Languages:
English; call for other languages.
Eligibility:
Serves residents of Nassau County age 18 or older with a household income at or below 200% FPL. Must provide proof of income in the form of either check stubs, most recent income tax, or notarized letter from an employer.
