Description:
Provides a comprehensive medical home for youth ages 16-26 with chronic medical or developmental problems. The staff includes a pediatrician, adult internal medicine specialist, nurse care coordinators, and a transition specialist who provide primary medical care to address the young person's immediate medical needs.
Hours:
Monday: 01:00 pm-04:00 pm, Wednesday: 08:30 am-12:00 Noon, Thursday: 01:00 pm-04:00 pm.
Intake Process:
Call for information.
Program Fees:
Accepts most insurance plans, including Medicaid. An income-based sliding scale is available.
Languages:
English and Spanish.
Eligibility:
Serves Baker, Clay, Duval, Nassau, and St. Johns county youth and young adults ages 16 through 26 with chronic medical or developmental problems.
