Provides comprehensive dental services for adults and children in the Jacksonville area. Adult dental services are located at the Downtown location, pediatric dental care is at the Sulzbacher Village. The care team includes a combination of dental hygienists and assistants, dental students, and volunteer dental professionals from throughout the community.
Hours:
Monday-Thursday: 08:00am-04:00pm and Friday: 08:00am to 12:00Noon.
Intake Process:
Sulzbacher Center residents should request dental service referral form from their case manager. Non-residents must call for an appointment.
Program Fees:
No cost for Sulzbacher Center Resident; Federal Poverty Guideline (FPL) based sliding scale for non-residential Sulzbacher clients
