Operates the Early Steps program for families with infants and toddlers who have developmental delays or an established condition likely to result in a developmental delay; each family receives an Individualized Family Support Plan (IFSP) that meets his or her unique needs.
Hours:
Monday-Friday: 08:00 am-04:30 pm.
Intake Process:
An initial assessment is required.
Program Fees:
None for initial evaluation and service coordination. Medicaid and most major medical insurance plans accepted.
Languages:
English.
Eligibility:
Children residing in Baker, Clay, Duval, Nassau, and St. Johns counties age birth to 3 years of age and suspected of having developmental delay or other established medical condition which places them at-risk for developmental disabilities.
