Offers outpatient education and training to individuals with diabetes or pre-diabetes for diabetes management in order to manage sugar levels and avoid complications. Services include nutrition counseling and Diabetes Self-Management Education Classes, including information about blood-glucose monitoring, meal planning, physical activity, diabetes medication, eating out, self-care, and preventing complications.

Hours:

Monday-Friday: 08:00 am-05:00 pm.

Intake Process:

Physician referral required.

Program Fees:

Accepts most insurance, including Medicaid and Medicare. An income-based sliding scale is available.

Languages:

English and Spanish.
