Services include nutrition counseling and Diabetes Self-Management Education Classes, including information about blood-glucose monitoring, meal planning, physical activity, diabetes medication, eating out, self-care, and preventing complications.

Monday-Friday: 09:00 am-05:00 pm.
Intake Process:
Physician referral required.
Program Fees:
Accepts most insurance, including Medicaid and Medicare. An income-based sliding scale is available.

English; written material in other languages are available.
