Taxpayer is experiencing financial difficulty or hardship due to an issue with the IRS, and the issue needs to be resolved quickly to avoid the hardship becoming more serious.
Taxpayer is dealing with multiple IRS departments and coordination is needed to avoid confusion and make sure everyone does their part.
Taxpayer has tried to resolve an issue through normal IRS channels but those channels have broken down.
