Description:

A free health clinic that provides emergency dental services, primary care medical services, as well as mental health counseling services for uninsured individuals who meet the financial criteria. They also provide free pregnancy testing and counseling, new born and infant services at Baby Luv.

Hours:

Mon-Fri - 9:00am to 2:00pm

Intake Process:

Due to COVID-19; medical and dental services are by appointment only.
Applications can be picked up outside the clinic.
Submit a completed qualification form before medical or dental treatment is offered.
Seeing dental patients on an emergency basis and medical patients via telehealth.
Photo ID is required.
Proof of income with application.
Letter of support.
Verification if receiving SNAP benefits, unemployment, one month pay-stub, child support, alimony, SS (retirement or disability), Veteran disability or TANF.

Eligibility:

Must be a Duval County resident.
Cannot be homeless.
Must not have any medical or dental insurance including Medicaid.
At least 18 years of age.
Must meet income eligibility for Federal Poverty Guideline for current year.
