Provides advocacy, employment training, and living services for people with a variety of physical, developmental, and intellectual disabilities (Down Syndrome, autism, cerebral palsy, spina bifida, Prader!€“Willi syndrome, and other mental disabilities).

Hours:

Monday-Friday: 08:00 am-04:30 pm.

Intake Process:

Call for information.

Program Fees:

Accepts Medicaid and Social Security Disability. Private payment plans are also available to those not enrolled in Medicaid.

English.

Eligibility:

Serves Putnam County residents registered with the Agency for Persons with Disabilities (APD).
