Description:

Provides healthcare services to uninsured and underinsured individuals aged 21 and older living in Duval County. Services are available to the general public by appointment. and include basic primary care and physical examinations, prescription medications, laboratory testing, diagnostic testing, specialty referrals, cancer screening, and patient education. Patients are required to provide proof of denial of coverage by Medicaid, and a monthly gross income at or below 200% of the Federal Poverty Level by producing two paycheck stubs as proof of income, if employed, or a notarized Letter of Support, if unemployed.

Open on the second Saturay of every month from 9:00am to 1:00pm
