Description:

Provides no-cost emergency dental services, primary care medical services, as well as mental health counseling services for uninsured residents of Duval County who meet the financial criteria. They provide free pregnancy testing and counseling, new born and infant services at Baby Luv, laboratory and diagnostic services.

Hours:

Mon-Tue-Thu - 9:00am to 4:00pm
Wed and Fri - 9:00am to Noon

Intake Process:

Medical and Dental services are by appointment only
Must submit a completed qualification form before medical or dental treatment is offered

Proof of income with application
Verification if receiving SNAP benefits, Unemployment, one month pay-stub, child support, alimony, SS (retirement or disability) Veteran disability or TANF
