They provide services that help people improve their daily functioning and live independently in the community. This service include assessment, service planning, linking and referring, monitoring, advocating, coordinating services, 24 hours on-call coverage, crisis support, and skills training in the home to promote independent living, where appropriate.

Mon-Fri - 8:00am to 5:00pm
