Description:
Mental Health Resource Center Therapeutic Family Program is designed to serve adults who have not been able to live independently in the community. All persons enrolled in the program reside with an adult foster family and provided intensive targeted case management services.
Hours:
Mon-Fri - 8:00am to 5:00pm
Intake Process:
Call for initial Intake and Screening Assessment
Program Fees:
Case management services covered by Medicaid; call for additional information
Languages:
English; other language interpreter service available as needed
Eligibility:
The program serves adults (18 and over) with severe and persistent mental illness
Must meet eligibility requirements
