Description:

Provides emergency financial assistance for utility costs to income-qualified families living in Putnam County. Payment is made directly to the utility provider, when funds are available. The automated appointment hotline is available 24 hours a day, seven days a week, and a live Operator is available Monday through Friday from 10:00am to 5:00pm. Language accommodations are also available.

Intake Process:

(Due to COVID-19 no walk-ins are permitted.)
Appointment is needed first for assistance; then the application should be completed.
Clients that live in Clay County must complete the City of Green Cove Springs Utility Form.
Appointments can be scheduled up to 14 calendar days in advance.
Appointments can be made through the Online Appointment Scheduler by phone.
Required documents can be scanned or pictures of the documents can be uploaded.
Utility Bill Certification Form must be completed if the utility bill is not in the customer.
Social Security number for all members of the household are required.
Household members age 18 and over with no income must complete the Self-Declaration Form.
