Provides medical services to uninsured patients at little or no cost. These services include orthopedics, oncology, dermatology, cardiology, urology, gastroenterology and others. Services include chronic disease management, vision screening, hearing screening and Gerri!s Corner Health
Hours:
Mon, Tue and Thurs - 8:00am to 5:00pm
Wed - 8:00am to Noon
Fri - 8:00am to 2:00pm
Intake Process:
An application will be required, appointments preferred for medical services
Walk-in for Urgent Care clinic
Program Fees:
Based on Federal Poverty Guideline at or below 200 percent for the current year
Languages:
English; call for other languages
Eligibility:
Proof of Nassau County residency
Proof of income: check stubs, most recent income tax, or notarized letter from your employer
18 years of age and over
At or below 200 percent of the Federal Poverty Guideline for current year
No insurance, or Medicare
Accepts Florida Medicaid
