Description:

Provides basic health care, including physical examinations, immunizations, family planning, nutrition assistance, and diagnosis and treatment of common ailments for low income individuals. Social and medical services are available to individuals in need, regardless of age, race, ethnicity, disability, sexual orientation, political affiliation, or religious beliefs.

Tue and Thu 8:30am to 1:00 pm
Fri 2:30pm to 5:00pm
Sun 12:30pm to 4:00pm

At or below 200 percent of Federal Poverty Guideline for current year
