Description:

Provides a free health clinic, including emergency dental services, primary care medical services, as well as mental health counseling services for uninsured individuals. Offers both in-person and telehealth services.

Hours:

Monday - Friday: 09:00 am-01:00 pm and 02:00 pm-04:00 pm. Once established as a patient, ongoing care visits may be schedulable outside of these hours.

Intake Process:

Call for information. A completed application will be required before services are provided.

Eligibility:

Serves uninsured individuals age 18 or older residing in Duval County. Cannot be homeless. Photo ID is required.
Must also provide proof of income, a letter of support, verification of all public benefits being received, including SNAP benefits, unemployment, one-month pay stub, child support, alimony, SS (retirement or disability), or Veteran disability or TANF.
