Description:

Provides a full range of supportive services for terminally ill individuals who are in the final stages of their illnesses and for their families. Services include medical care, pain and symptom management, case management, emotional support, palliative care, advance care planning, and bereavement services for the patient and members of the family.

Hours:

24 hours a day, seven days a week.

Intake Process:

Generally requires a referral from client's current physician or care team; must meet general hospice eligibility and general inpatient criteria.

Program Fees:

Accepts most insurance, including Medicaid and Medicare. An income-based sliding scale is available, and service is rendered regardless of an individual's ability to pay.

Languages:

English.

Eligibility:

Serves residents of Duval County.
