Provides customized outpatient therapeutic programs and Medication-Assisted Treatment (MAT) programs to individuals age 18 and older with an Opioid Use Disorder and who are experiencing active addiction to heroin, fentanyl, or other prescription pain medications. Services are offered to residents of Orange Park and the surrounding Clay County areas including Fleming Island, Lakeside, Middleburg and Green Cove Springs. Treatment combines behavioral counseling with stabilizing medication, such as Methadone, Buprenorphine, or Suboxone, depending on which substance(s) an individual is addicted to, and for how long. Additional services include harm reduction for individuals in active addiction, medically supervised withdrawal, medical exams to ensure safety, convenient outpatient services, case management, confidentiality, referrals to other community resources, and take-home benefits after meeting key criteria. Narcan distribution is available to the public. Individuals seeking to become a new patient can call the toll-free hotline 24 hours a day, seven days a week to schedule an appointment.

Intake Process:
Patient needs to be in withdrawal at the time of their first appointment. New patients will meet with the doctor, a dedicated counselor and a nurse to receive their initial medication.
