Provides a full range of inpatient services on an ability to pay basis and target low income and indigent residents of the community. Services include physical examinations, immunizations, family planning, nutrition assistance and diagnosis and treatment of common ailments.

Hours:

Monday-Friday: 08:00 am-05:00 pm.

Intake Process:

Appointment preferred; walk-ins are seen on a first-come, first-served basis.

Program Fees:

Accepts most insurance, including Medicaid and Medicare. An income-based sliding scale is available.

English.

Serves Duval County residents.
