Description:
Offers mental health counseling, substance use disorder counseling, medication assisted treatment, and treatment for adults, teens, and children in Clay County (Green Cove Springs, Middleburg, and Orange Park). Intensive family services include parent education and support for parenting techniques, family preservation services to keep children in their homes, family reunification, case management support, and in-person crisis intervention. Mobile Response team provides services to children and adults age 25 and under. Outpatient counseling and treatment, onsite service in some schools, and walk-in services available at select locations.

Mon-Fri - 8:00am to 5:00pm
Intake Process:
Walk-ins accepted Mon-Fri - 8:30am to 2:30pm.
