Description:

They provide routine health care provided by family medicine specialists, gynecologists, obstetricians, nurse
practitioners, midwives, physicians' assistants or other health care providers who serve as the first point of contact with the health care system. They provide basic medical services for uninsured residents and does not provide emergency services.

Hours:

Mon, Tue and Thu - 9:00am to 5:00pm

Intake Process:

The clinic will remain open to existing patients Mon, Tue and Thu 9am-5pm. The clinic is not taking new patients until further notice. Hours may be modified without notice.

Uninsured Clay County Resident who live below 200% of the Federal Poverty Guideline for current year.
