Description:

Provides programs that impact struggling and at-risk students through case management, literacy tutoring, afterschool programs, and summer camps. Services are offered in select Duval County Public Schools and vary by school and grade level or age of students. Afterschool programs are held Monday through Friday for three hours after school wherein students receive academic support from certified teachers and participate in enrichment activities such as robotics, symphony, sports, life skills, and more. Students also receive a snack and a hot meal. The Hope = Success High School Afterschool Program takes place after school, three days per week for two hours and teaches high school students about workforce development, entrepreneurship, employability, college preparation, financial literacy, healthy relationships, and more. Also offered for high school students is a six-week summer camp for students Monday through Thursday from 9:00 am to 1:00pm.

Students and families are helped to overcome obstacles that range from learning disabilities, social struggles, food insecurity and homelessness.

Duval Reads Literacy Program- The Duval Reads program provides intensive one-on-one and small-group literacy tutoring for students during the school day and in the afterschool program.

Case Management-Achievers For Life- Achievers For Life (AFL) is a dropout prevention initiative developed and funded by United Way of Northeast Florida in partnership with Communities In Schools of Jacksonville and Jewish Family and Community Services. AFL focuses on sixth-grade students showing risk factors for school disengagement. These factors include low GPAs, low FCAT scores, and excessive absences, and provide them with family support, a mentor, and in-school assistance. Our Communities In Schools achievement advocates work closely with these students to keep them on the right track and prepare them for high school.

Case Management-Bridge to Success- the Bridge to Success Program is dedicated to students over-age for their grade level. Our site coordinators address all barriers to graduation and provide the necessary support to help these students graduate on time, ready to fulfill their college and career aspirations.

Case Management-GEAR UP- These transformative programs start with 7th-grade cohorts in 10 DCPS middle schools, followed by 7 high schools. The grants are cohort-based, allowing us to follow these students through high school and one year of post-graduation transition. Communities In Schools case manages groups of students at each participating school.

Case Management-The Student Enrichment Program- This program places full-time site coordinators on school campuses across Duval County, working individually with a caseload of 75 students. These students receive individual integrated student support services and in group sessions throughout the year. Our goal is to empower them to achieve their dreams while equipping them with the essential life skills they need to succeed. Home visits and parental involvement are essential parts of the program.
