Description:
Provides transitional housing services for low-income male Veterans experiencing homelessness, with the goal of transitioning to permanent housing and independent living within 24 months. Residents are required to participate in case-management and life-skills classes, complete a mental-health assessment, and engage in mental health counseling, complete a health assessment, maintain sobriety, and participate in substance-abuse education classes and support groups.
Hours:
Monday-Friday: 08:30 am-05:00 pm.
Intake Process:
Requires referral from Changing Homelessness and Veteran Affairs Community Resource and Referral Center.

Must be a Veteran located in Duval County. DD-214 required.
